Why Sign In

Sequentia enables you to search for potential care providers on behalf of yourself or a loved one. By simply registering with your name and email address, and then signing in, you will be able to Save and Share your search results. Your My Sequentia Folder will contain the Profiles of the Care Providers that you select from your search.

If you find yourself in need of searching for care providers for another individual you may add them here. After you log in you can select the individual you are searching on behalf of by selecting them from the drop down list in the upper left of your screen.

Why Share

Whether you are searching for an extended care provider for yourself or a loved one, you can easily share your search results with family members and others involved in the selection decision. Simply enter their name and email address and save the list.

Once you save the names and email addresses of those you choose to share with, an email will be sent to each with a link back to your Folder. Each person is then able to view the profiles of the selected providers in the list, and to also search for and add more potential care providers to the list.

Why Hospital Connect

This functionality of for hospital discharge planners and case workers that manage applications to care providers on the patients behalf. It allows them to sign into their Sequentia portal and copy your care provider selections.

Why Add "Search On Behalf of"

If you find yourself in need of searching for care providers for another individual you may add them by clicking on the Add link. After you log in you can work on behalf of the individual by selecting them from the drop down list in the upper left of your screen. This drop down becomes important when you are searching on behalf of more than one person.
